Microsoft Access Task

I am looking for a database expert to create a simple doctors office database, and explain how the database function by providing the following:

Describe the data modeling stage and the steps you are taking to create the data model.
•  Provide screen shots of the database design tool and implementation.
•  Identify the queries that you will be running and the need for the queries.
•  Include the data modeling layout including the E/R diagram and other specifications.

•  An outline of your work which discusses the database structure and
implementation, and includes snapshots of the tables, relationships, and
queries.
•  Identification of the snapshots of the relationships
between tables and the type of relationship: one-to-one, one-to-many or
many-to-many.
